Get an eye exam from your eye care practitioner.

Buy 8 boxes of Acuvue Oasys for Presbyopia Lenses.

Complete the online form on acuvue.com
and print the Rebate Certificate.



Attach the following:

- Two Acuvue Oasys for Presbyopia box tops
- A receipt of your eye exam
- The product purchase receipt.
